To register with the practice please complete the application form and return to the surgery.

We are accepting new patients here at The Saddlers Health Centre, you can register with the practice online through our NHS site or simply collect the registration forms from our reception team. We ask that you complete all forms to the best of your knowledge providing Photo ID and proof of address if you have this available. If you have your NHS number please include this on your registration form as this will be a easier and quicker way to register to our system and means that we are able to obtain information from your previous GP through the provided links. Please make known on your registration form if you first language if not English this way this will help us identify any translation needs you may have/need.
If you are a new patient who is currently pregnant please make sure you make the staff aware of how far in your pregnancy you are so that we can arrange the necessary referrals with your registration with the practice.
Once your registration has been accepted you will be asked to book a New Patient Health Check with the HCA speak to the reception team to arrange this.
We make every effort to accommodate all new patients no matter what background.
Everyone in the UK has the right to free care from a GP. If you don’t have proof of address, ID card or and address you can still register with the practice. Immigration status or nationality don’t matter – reception staff will only ask your immigration status and will not share your information with the Home Office unless a serious crime is involved. New To This Country
We are also registering those patients who are new to the country if this be work purposes or who are asylum seekers. We understand that ID may not be available for some registrations and are still able to accept registrations for people in certain criteria. Please speak to the reception staff if you have any questions.
Named GP
All patients on registration are allocated a named GP. All patients are advised of their allocated named GP via text message confirming their registrations acceptance.
Patient do have a choice of what GP they see Male Dr Mahmood, Female Dr Verma. The surgery will do their upmost to support your preference but it is not always possible. Both Dr Mahmood and Dr Verma will be directly and indirectly involved in your care.
Catchment Area
The practice is located in central Walsall and we accept patients who are in the Walsall area. We do have patients who reside in other parts of the area. In general, patients are accepted with the following areas, radiating out from central Walsall: If you are registering and live outside of the catchment area, your registration will be reviewed with the GP and a decision will be made. Please note we do not provide a home visit service to those patients who reside outside of our catchment area, this will be discussed with you at the time of registration.

Up to and including Yew Tree Estate, Walsall
Up to and including Bescot Road, (Junction 9 M6) Walsall

Up to and including Mossley and Beechdale Estates
All Palfrey, Caldmore, Delves, Pleck, Chuckery, Hatherton, Goscote, Leamore and Butts areas.
