Registration with a GP

NEW PATIENT REGISTRATION FULL PACK – OVER 16

Patients wishing to register with one of our G.P.s, should request the appropriate forms at the reception desk. They will be asked to complete three forms, which should then be handed back to reception staff. You will then be given an appointment to see our Health Care Assistant . During this appointment you will be asked for information about your health, family history and your lifestyle. It is important that we have this information to assess any medication or future health problem.

Once you have been accepted on to our list, full registration will take place and your old G.P. will be notified of your request to change doctors. Please remember that it could take one month for your full medical records to be transferred from your old G.P. In some cases it may take longer.

We make every effort to accommodate all new patients, but if the G.P.s feel that they are not sufficiently trained to deal with your problem, it may be necessary to suggest that you register with another G.P.

Coronavirus (Covid-19)

 
Stay at home for 7 days if you have either:
 
• a high temperature
• a new continuous cough
 
Do not go to a GP surgery, pharmacy or hospital.
 
You do not need to contact 111 to tell them you’re staying at home.
 
Use the NHS 111 online coronavirus service if:
 
• you feel you cannot cope with your symptoms at home
• your condition gets worse
• your symptoms do not get better after 7 days
 
Only call 111 if you cannot get help online.
